The shoulder joint moves in a circumductive pattern, combining flexion, extension, abduction, and rotation. The wheel provides the most effective example of this motion. By rotating the wheel, users activate the shoulder through its full natural range.

Designers modeled the shoulder wheel after the spoked helm used on sailing boats. Therapists mount the wheel on a wall to allow full shoulder elevation and smooth circular movement.

Adjustable Resistance for Progressive Therapy

Many shoulder wheels include a thumb screw mechanism. Therapists tighten this screw against a brake drum to adjust resistance. This feature allows gradual progression based on the client’s strength and rehabilitation stage.

Clinical History and Use

Rehabilitation gyms widely adopted the shoulder wheel long before formal documentation appeared in clinical literature. Although early rehabilitation texts did not mention it, Elizabeth McClusky published the first detailed work on shoulder wheel use in 1958, followed by references in major rehabilitation texts in 1961.

Today, therapists continue to rely on the shoulder wheel as a reliable and effective tool for upper limb rehabilitation.
